The Wrong Con

While most people with mood disorders don’t technically “hear voices,” they sometimes experience something much scarier; their own internal monologue lying to them. Whether your mental illness is convincing you you’re worthless or the king of the world, don’t follow your corrupted instincts when it comes to managing medications and drawing up safety plans. Instead, study the facts, learn from your own experience, and plan the odds, rather than letting your confidence play you and make you crazier than you really are.

I am a doctoral candidate preparing to defend and graduate in May. I am so terrified of what comes next (giant black hole; there be dragons) that I am undermining my progress and succumbing to despair. This is tied to my utter lack of ambition. I don’t know how to dream Real Things. All this worry and fear and lethargy leaves me feeling exhausted. And I am so worried and so frustrated with (what I perceive to be) my laziness, my incompetency that I become angry at the smallest things. How can I learn to get work done and to build a life? How can I learn to do it just because it must be done? How can I distinguish between real and false selfishness (it feels selfish to build a life just for me)?

The business of many doctoral candidates is to find a meaning in many random, painstakingly-researched, frustrating phenomena that ties them together and gives you a new idea you can dwell on for several hundred pages. Congratulations, you’re very good at it; too good at it, as you’ve turned your talents on your own troubles.

It’s certainly possible that your fear of any and all future jobs, combined with terminal laziness, incompetence, and an inability to distinguish between real and false selfishness have paralyzed you and filled you with self-loathing. Value Added Facts

If everything that was good for you also felt good for you then kale would be heroin, running would be orgasmic, and one-a-day vitamins would put you into a pleasure coma. Unfortunately, bad things often feel best, which is why heroin is heroin, kale tastes like land seaweed, and passion can be poisonous. That’s why you can love someone who’s just not good for you and hate a job that you do well for good reasons, but before assuming your feelings are telling you the truth, take time to measure a relationship by how well it fulfills your purpose, meets your standards, and satisfies your moral priorities. Then you can do the right thing, even if it feels (or tastes) terrible.

What could have been a perfect relationship slipped very quickly down hill as two insecure people who have both been emotionally abused by our families growing up both went through stressful times suddenly. We couldn’t manage to make it through the bad times due to coping mechanisms we both employed to save ourselves from more pain, having not had long enough to make the relationship secure. Still, I’m really struggling to let him go. I felt this connection that I’ve never felt before and this is the first person I’ve ever missed in my life. I know he needs space to sort himself out but I want him back and I’m not sure whether to cut the cord now even though I really feel like I can’t and it would cause more pain. I don’t know how to let him go, or make the right decision.

The strength of your connection to a lover is great inspiration for a love song—maybe something by Taylor Swift if she ever dates Sean Penn—but a good song won’t tell you whether a relationship is good for you, is likely to last, or what you can or should do if it falters.

The fact that you and your former lover are insecure victims of abusive families may explain why you’re both anxious and vulnerable to doing negative things when you’re scared, but it won’t tell you how much he can control his negative behaviors and/or tolerate yours. Job Lurch

There’s no perfect way to deal with someone close to you when, due to depression, discouragement, and/or plain laziness, they become too dependent on you for support; whether your strategy is to give loving comfort or a tough kick in the pants, their attitude tends to defeat them and their would-be helpers. If you wish to help someone who is under-engaged in life, whether your motivation is love or self-survival, don’t wait for the underactive to feel rejuvenated. Instead, create a regimen and reward progress with gifts, food, and/or good ol’ verbal encouragement. Then, if they still don’t do their share, decide on the support you think is right to provide, knowing that change is not an option, but cutting them off is.

My younger son is 23 and living at home. He dropped out of university and has drifted since, working abroad for a while, now doing an office job on a temp contract for the past year. When not at work he sleeps in and is lackadaisical in direction. I am going through a divorce from his father and working very hard in a low-paying job. I am also having to support my daughter through the collapse of her marriage. My son wants to quit his job which he dislikes and sees as a dead end and use the time to pursue a career. His earnings are paying for some of his bills and he hasn’t saved much although I charge very little rent. I fear his half-hearted approach will soon return and resent coming home from a tough shift at work to find him still in bed. My goal is to be supportive without feeling used and resentful. I am worried about him but nagging wears me out and I have problems of my own.

Whatever career your son hopes to pursue, let’s hope it’s not one that requires basic skills of observation; you’re obviously stretched out, so if he thinks he can rely on you when he doesn’t have income, he’s not just oblivious to your situation, but his own.

Even if he doesn’t realize it, you know that his business plan is heavy on dreams and light on discipline. Instead of expressing anger, skepticism, or disappointment, however, ask him whether he sees himself as having a problem with avoidance. Shame may have made him lie about his difficulties and pretend that he’s done more than he has. If he can acknowledge the problem, you can offer him coaching, advice, and incentives for building good habits. Crazy, In Love

In the self-help-iverse, “limits” is often a dirty word, because we should all aspire to reach the loftiest heights or depths of intimacy. In reality, limits are crucial when setting both personal and interpersonal goals, but they’re especially vital when you’ve got a partly functional, mentally ill family member to take care of. In that situation, you will probably feel like doing anything to get him/her into treatment, including persuasion, confrontation, and threats of expulsion. In reality, your influence over the course of a mental illness is often, well, limited, treatment or no, and trying too hard to make it better can make it worse and drive you crazy. Instead of getting over-absorbed in efforts to help, get help yourself in figuring out the limits of what’s possible and respecting your other priorities, including safety, security, and the nurturing of others. Refusing to acknowledge limits is a lot easier/more damaging that learning to respect and use them to your advantage.

I’m married to a wonderful, unique individual, a free spirit who lived in a van for several years avoiding the world. After we married, we had a difficult couple of years, but then my work took off, and I got pregnant. My husband, already a night owl, turned into an irritable, stressed out insomniac who oscillates between manic episodes of ultra productivity, to sleeping for marathon amounts of time, missing appointments, being late for work, generally letting everyone down. He seems so resentful of me—everything I say he takes to the utmost extreme. We have been in talk therapy for three months, which has been pretty useless. The therapist has recommended my husband see a general doctor and a shrink about the insomnia. On a long road trip he started hallucinating and driving off the road. Sometimes he admits there’s a problem, other times he doesn’t. This morning, after another night of no sleeping, he screamed at me, collapsed on the floor and then crawled into the closet and passed out. I have asked him to leave our house until he seeks help, but he also refuses to leave. If I don’t pay the bills, he will be unable to. I reached out to his family and they keep saying it’s a marital tiff. He’s a wonderful man who is deeply troubled right now and in huge denial. I think he has some serious mental health issues which have been brought up by the huge responsibility of being a father which need addressing. How can I best help him?

You know that your husband’s dangerous behavior is out of his hands and in his mind. Unfortunately, there are lots of medical and mental health problems for which no one, including this mental health professional, has the answers, and this may be one of them.

The danger of trying to help him if it’s just not possible is that you’ll exhaust yourself and, worse yet, put yourself and your baby at risk from his symptoms. His illness may not be his fault, but that doesn’t make it your only responsibility, either. WAIT! There is more to read… read on »

Therapy, You and Me

Whether you believe in a particular psychotherapy or find the very prospect of head-shrinkery scary, don’t let the emotions that draw you towards or away from the therapist’s couch drive your treatment decisions. Develop your own fact-based procedures for deciding whether you’re fucked-up enough to need therapy and, if so, whether there’s a treatment that isn’t too risky and has a good enough track record. Maybe we’d all like the experts to figure that out and make such decisions for us, but we’re better off picking the brains of experts and then making treatment decisions, pro or con, for ourselves.

How long is too long to be in therapy? I have been in therapy for four years now, and it has helped enormously. I went into therapy initially because of a trauma situation, but I don’t want to stop, even though I’ve long worked through that trauma, because I still think I can benefit. Still, now that I reached the four-year mark, I’m wondering, how long is too long?

If you’ve read this blog, you’re probably aware of our belief that the most important goal of mental health treatment is seldom to relieve all your pain; that’s usually an impossible pursuit, or one that just shifts the pain from your head to your wallet and your friends, who are sick of hearing about what you learned in therapy.

Diagnosis Focus

It’s easy to know and describe what it’s like to be physically out of sorts—usually, Nyquil or a triage kit are in order—but when you’re losing it mentally, things get a lot more complicated. In some cases, it’s uncomfortable and can’t go away soon enough, while for others, it’s great and not something they even see as a problem. Good or bad, however, it’s easier to identify and understand, at least to the person experiencing it, if you remind yourself that a state of mind is only a state of mind. So whether or not you can change it, or believe it’s worth changing, it’s always worth remembering that there are more important things. Your job is to make the most of your state of mind, even if you can never fully make sense of it, without letting your brain run your life (and body) off the rails.

I don’t know how to begin…It’s really strange because I’ve never felt like talking to a psychiatrist, but now that I’m trying to, I realize how much I may need one. I’ve been trying to find one of my, of what I realize now, many fucked up aspects to talk about. Why do we bottle things up? Why do we make the bad things the deepest parts of our lives? Happy moments are like listening to the Beatles, short and like being on acid or running through a meadow…or both. But melancholy sits inside like Joni Mitchell or Jeff Buckley, if you let it. I guess my point is that I am terrified that I am going to have Virginia Woolf or Sylvia Plath’s life. Almost everyone in my family has mental issues. My brother tried to kill himself this year, my grandparents are the lovable nut-bags, my father had anger issues during my childhood… and I got the diagnosis from my doctor this year that I am depressive with bipolar tendencies. It’s terrifying to get a name for the way you are. I have so many. I wish it was duchess sunshine awesome, but you know… I guess what I came here for was to say that I don’t know how to express my emotions, should I bottle them up? Or should I just let myself go? My goal is to be a little clearer.

We don’t have the power to diagnose people over the internet, but when we get letters from people fretting over psychiatric diagnoses we usually feel comfortable diagnosing those diagnoses.

So, assuming you do have the familial tendency to be depressed, have mood swings, and, presumably, be creative, you don’t have to have a fucked-up life, nor do you have to stifle your creativity.

What you will have to do, however, is work at keeping your perspective when you’re hurting with depression and everything seems to suck while not letting your diagnosis frighten or shock you. After all, a doctor has merely put a label on what was always there, meaning you now know there are techniques that can help you deal with what’s been ailing you. WAIT! Hope Springs Infernal

The phrase, “when the going gets tough, the tough get going,” is a bit unclear; after all, in a tough situation, you can either buckle down and get more focused or just give up and get going outta there. Trying harder or just giving up both give us a sense of control, but no matter where the tough go, either choice can easily make things worse. So when life becomes difficult, first assess the odds that more work will yield more positive results. Then, if there’s no way out of a tough spot, respect yourself for what it takes to eat shit and stay put. And no matter what, always make careful use of your clichés.

I know my adult daughter is a terrible avoider and procrastinator—she struggled all throughout school, and only graduated from college with a lot of help—but she made great progress a year ago after we developed a schedule and daily to-do list, and she was very good about using me as her coach. After her last lay-off, however, she lost steam and started looking depressed—she stopped getting up on time, keeping up with her calls, getting exercise, going through her mail, etc. She’s not totally incapacitated and we still talk every day, but she’s vague about what she’s actually accomplished. I’m thinking we should start over and develop a new system for evaluating what she needs to do and tracking whether she’s done it. When I talk with her about starting a new system, however, she gives me more vague answers, which leaves me frustrated and very, very worried. I’ve got to find some way to push her or she’ll never get going and then it will become more and more difficult for her to resume her career. She’s almost 30, and I can’t push her forever.

Throughout time, humans have developed useless-yet-comforting rituals to ease us through hopeless situations; from ducking and covering through a nuclear blast to stocking up on duct tape during an orange alert, there’s nothing like an empty ritual to distract you from the fact that, ultimately, you’re totally fucked.

As such, the good news is, you’ve done everything right by your daughter to help her manage procrastination by drawing up a schedule and to-do list and offering to track them with her. The bad news is that all those positive steps sometimes lead nowhere, and trying to figure out a new system at such a time might be about a useful as hiding under a desk to avoid a mushroom cloud. Coerced Depression

Saying that being depressed just means being sad is like saying that having cancer means feeling nauseous; depression involves a lot more than misery, including anxiety, self-doubt, exhaustion, physical pain, and even nausea, depending on the individual. Some people are always at least a little depressed because they’re never satisfied with themselves, and others have severe symptoms but only briefly after special disappointments. In either case, until depression gets it’s own chemo-like treatment, people can seldom cure their symptoms. They can, however, learn to think more positively about their lives while not confusing the pain of symptoms with failure or character defects, or the pain of life with depression in general.

I have a very demanding and high pressure job, part of which is done in public, so when I make a mistake, I can’t stop thinking about it or beating myself up. I always think the worst, even for the minor stuff, but I don’t understand why. I accept mistakes in others, why can’t I accept them in myself? I am not arrogant or self-important; just the opposite, I often think I’m the dumbest, most inept person in the room. I’ve had some success but I always attribute that to external forces, not anything I’ve done. This is starting to really affect my life. I cry a lot. I don’t know what to do. Please help.

The fact that you got such a demanding and high pressure/profile job probably means that you have what it takes to get it done, but part of what helped you get it and do it well is being ultra-conscientious, which means worrying, self-criticism, and now, probably, depression. Circle of life, meet circle of strife.

The good side of worry and anxiety is that they drive you to work harder and look out for mistakes. Worriers succeed, not to keep my industry alive, but because worry-genes help people survive and multiply. Magical Drinking

When it comes to drinking, or really any addiction, it’s hard to stop without a “good reason,” especially if you think there’s a better reason not to quit. Then there’s the expectation that, if you really need to stop, the good reason fairy will visit you in the middle of the night to let you know (usually while you’re sleeping in a stranger’s bed, in a dark alley, or a puddle of puke). In reality, it’s up to the drinker to decide if s/he has to stop, and rational thinking about drinking doesn’t require a degree in addictionology. All you need is discipline to gather facts, courage to look at them, and determination to use good reasoning to do what you think is best for yourself.

I’ve lived alone since my daughter left for college (my wife died years before) and I wonder if I’m drinking too much. I’m good enough at my job, but the head of our office doesn’t respect me and I wonder if I’ll have to move on. I’ve always had a tendency to get depressed and I see a therapist, but my antidepressant medication doesn’t seem to be working. I don’t sleep well. I’m not sure drinking is doing any harm and it certainly eases the pain, but I do get completely drunk every night, and it’s become the highlight of my day. I’ve got no friends currently, but I’ve never been a sociable guy so it doesn’t interfere with my social life, and since I’m alone, I get no complaints from friends or family. No harm done. I’m healthy and my hangover isn’t bad, so I wonder whether my drinking is worth worrying about, or whether I should just focus on getting help for my depression.

To paraphrase the old koan, if a person falls into excessive drinking without anyone around to become concerned, does it make that person a drunk? Nobody can ask the tree if it thinks it made a sound, but since you’re a person, you’re not just able, but the only one qualified, to answer the question.

You might be persuaded that you’re drinking too much if a therapist suggested you were using it to escape painful feelings, or if a spouse complained, but then later on you might decide that there’s nothing wrong with escaping when life sucks and your spouse has no right to complain because her nagging drives you to it. Nature Versus Searcher

As we often say, approaching dating as a “search for love” is like trying to keep people safe by starting a “war on terror;” since you can’t date love and you can’t kill hate, your quest is likely to be frustrating. Sometimes people doing a mate-search have a good idea about what to look for but don’t have the good work habits they need for the job. Others with fine work habits get staggered when the carefully chosen prince or princess they kiss turns out to be a frog. Remember, unless you’re very lucky, any search requires both a disciplined method and an acceptance of the fact that good matches are hard to find. The less romantic you are in your methods, the more romantic you can let yourself feel later on, but at the outset, figure out exactly who, not what, you’re looking for, in order to have good—or any—results.

I’ve struggled with depression several times in the past (that I’ve gotten out of through exercise, counseling and little cognitive therapy workbooks), so sad feelings are hardly new to me. But the sudden way they come about lately has me really freaked out. I actually like my job. If I let my work pay for more schooling, and I stick with it, I could make a really good life for myself. These mood swings seem to mostly exaggerate sad feelings I already have about not getting any and might be related to PMS, although I’ve never had period-related mood swings like this before in my life. Despite being an attractive, young girl that likes to go out and be social, I’ve never had a long-term boyfriend. I’m so frustrated that I can’t find a guy that I’m both attracted to and think is a good person, and that likes me back. (Habits like spending money on expensive clothes instead of student loans, and drinking lots on the weekends don’t help.) I eventually want babies (I think I would be a great mom), a partner, a garden, and to be a good person so my goal is to somehow control these mood swings, and maybe take online dating a little more seriously. I just want your opinion first.

From what you’ve said, my opinion isn’t far from your own; your values and goals are good, but your habits and mood swings aren’t. You’ve found a job you care about and want to get better at, but between dips of depression, drinking, and being distracted by the wrong guys, you’re stuck.

It’s not unusual for depression to push people into bad habits, like drinking and other feel-better-now-sorry-later activities, just in case the disease alone isn’t doing enough to make you feel like a pathetic loser who can’t get work done or have normal social relationships.

As you’re well aware, it takes time, lots of practice, and even worksheets to keep your perspective and hold your ground against an invasion of negative depressive thoughts.
