Some medical issues can be resolved quickly, but most of the serious ones aren’t that easy; for every temporary infection and sprain, there’s the eternity of diabetes and, of course, mental illness. Just because your crazy isn’t going away, however, doesn’t mean you can’t try to figure out how to lead a sane life anyway. And if you can get your crazy under control, you have to stay vigilant in order to keep it that way. So don’t try for control that is perfect or permanent; that’s as farfetched as a cure. Prepare to take one drug and one symptom at a time until you know what you have to deal with and what works best for the long run. Even if you can never cure the pain, you don’t have to let it be an overwhelming pain in the ass.

I’m not depressed any more, but going to the hospital and taking medication didn’t change the fact that my wife looks at me in a different way than she used to, and she spends more time at the gym, where there’s a handsome trainer who knows her name. She says I’m crazy and paranoid because this guy’s gay and just being friendly and, after twenty years of raising the kids, she’s too tired to mess around anyway, but I know what I see. And there have been signs in the way she seems happier and sweatier when she gets home from working out and her sweat smells more manly than feminine. My goal is to get someone to see that it’s more than coincidence, and that I have good reason to feel she can’t be trusted, and I’m not just nuts.
The paradox of feeling paranoid is that validating your fearful suspicions is what you both crave and dread the most. If you’re right, then you’re not crazy, but neither are your worst fears; your sanity may be intact, but your world would be destroyed.
Having those fears invalidated isn’t so hot, either, because it means that you can trust the world around you, but your own brain is suspect. So if proving and disproving your suspicions will always end badly, learn how to give less weight to those nagging thoughts in the first place. WAIT! There is more to read… read on »

If there was an ancient koan about self-destructive people, it would probably question whether the constant screw-ups are caused by making crazy decisions, or failing to make good ones (and maybe a leaf and stream would be involved). Unfortunately, some actively self-destructive people are the way they are, and some passive failures just can’t be who they should be. So, if you’re perplexed by someone who can’t do right or can’t avoid doing wrong, don’t assume you can help or that they can help themselves. Take a careful look at their ability to accept help and do better before deciding whether help is possible, blame deserved, and certain riddles (and people) would best be left alone.

My sister has always had a lot of struggles in her life, starting from when she came out at a young age and lost our mother soon after. My dad has given her a lot of support financially over the years and continues to do so, even though she’s got a partner (male) and a baby whom we all adore and look after any chance we get. Here is where the rest of the family and I are starting to get concerned; ever since my niece was born, my sister smokes pot daily with the baby right beside her, and recently she’s been making some rash decisions: breaking up with her partner who loves her and supports her; dating a woman with a history of being unstable and bringing her to their house; and planning to move out although she has no job/income. The whole situation is starting to take its toll on my aging dad (who is very involved in the care of her child) as well as the rest of the family, and if anyone tries to broach our concerns with her she explodes into a rage. Ultimately I think the root of the problem is the pot addiction (maybe combined with the anti-depressants), her unwillingness to quit smoking, and the fact that our nephew is affected. We are all at a loss as to what to do and how to approach it for fear of alienating her and thus our nephew, but we need to set some boundaries around our support for her. Or, do we stay out and let her live her life, meanwhile watching how it affects our nephew and divides the family? Our goal is to figure out how to offer help without getting cut off and heartbroken.
Unfortunately, you’re right in assuming that your sister will probably respond negatively to any limits you place on her. In short, if you imply that she’s fucking up her life, she’ll say fuck you, but with you and your family cut off, your niece will be extra fucked over.
The key to not making things worse as you try to help is remembering that some people are just like that and can’t be rescued. When it comes to navigating through problems in a relationship, you can’t always trust your ability to recognize the difference between major issues that can be worked through and those which mark the end of your journey altogether. Even if you can see what terrain lies ahead, there may still be no clear route to getting past the issue. So don’t assume you should have an intuitive relationship GPS to tell you how to overcome problems and know which are predictable or your fault. When things go wrong, re-analyze your plans and prepare to accept sudden changes to your destination.

After some rocky years, I’ve worked hard to build a supportive relationship with my son, but I’m worried about his new girlfriend. He’s crazy about her, and she seems to like him, but she was so surprisingly critical of him and everyone else at our first meeting that I left feeling very worried. He explained afterwards that she’s very sensitive because she was abused and that she’d made it clear to him that she was sorry that she had lashed out, but that didn’t do much to ease my concerns. Then again, if I asked him why she treats him badly and why he puts up with it, he would just stop talking to me. But if I say nothing, I’m worried I will lose him to a very unhappy relationship with a difficult woman. My goal is to save him from rejection or worse without possibly sacrificing what we’ve struggled to maintain.
Any time parents take on the responsibility of saving their kids, there’s often a huge sacrifice involved, i.e., a savings account, a kidney, or, if you’re literally taking a bullet for your kid, a pulse.
Thankfully, saving your kid from a bad relationship need not be your responsibility, nor must it require a huge sacrifice of any kind, from losing your life to your fragile relationship with your son. WAIT! There is more to read… read on »

We’ve written many times about the way mental health professionals especially tend to be either demonized or canonized; nobody expects their dentist to fix their lives or thinks their accountant is a monster and a fraud when s/he’s not perfect, but these are the expectations for those who deal in problems that are frightening and poorly understood, like mental illness. People would like to think therapists can provide control, but they’d also like to think the problem will go away by itself if you return to your usual routine. If you can accept the fact that some problems can’t be solved, however, and that the influence of professionals is always limited, you’ll be ready to learn everything you need to know and become your own expert on tough problems, imperfect professionals, and, if you’ve got the time, your own taxes.

My fifteen-year-old son does poorly in school whenever he gets depressed, which is fairly often, but his current school’s counseling staff is totally worthless—they haven’t just failed to help him, but so many students that their ineptitude is an open secret amongst parents and teachers—so I’m worried that they won’t do much for him once the depression starts and his grades slip. My goal is to figure out what to do to get his school to provide the counseling services he (and other kids) deserve.
If counseling were a reliably good treatment for depression and was available exclusively through schools, then you’d have a worthwhile fight on your hands. The movie version would win awards and you’d get your face on a dollar coin.
Unfortunately for your Oscar dreams, but fortunately for your son, the stakes for your battle aren’t nearly that high.
In reality, the help that almost all counseling provides is limited, and may have less to offer now that you and your son are knowledgeable about depression and can talk to one another about it. No matter what the talking heads say, a bleeding heart is not a partisan trait, nor is it always a negative one. You don’t even have to be a registered voter to be a good, caring person, and party affiliation doesn’t determine whether you’ll care too much and take responsibility for problems that you can’t really help. Learn how to assess your responsibilities realistically, whether you embrace or reject the problem at hand. Then, when a problem comes within range of your heart, you’ll be able to decide what to do without having to blindly follow any party line.

My girlfriend’s father is a widower in his mid-eighties who is still physically fit and able to drive. He is a difficult man, socially awkward and uneasy in company. He fills his days by going round thrift shops and yard sales buying old books and large quantities of stuff which he does not need or use. He used to sell it, but the dealers he supplied have died or long been retired so it just mounts up, particularly since his wife died. Now his house is a mess and a lot of living space is now uninhabitable. He cannot bathe or shower as the tubs are used to store stuff. My girlfriend feels guilty and stressed, but is too busy to do anything about it. I wonder whether I can move in with her if this is a family trait. I find this sort of lifestyle depressing and off putting. She is a kind and reliable person with many good qualities. My goal is to work out a coping strategy.
Caring about other people’s problems is a good trait if you can do something to help them, but otherwise it’s a good way to cause yourself trouble you don’t need. It’s just like hoarding, except with anxiety instead of expired food and dead cats.
Before taking on responsibility for an unsolvable problem, ask yourself whether that problem is likely to cause you trouble, or whether there’s anything that really needs to be done about it. Much is made of the inextricable link between trust and love, but the assumption seems to be that you can’t love someone else unless you can trust them (to listen, keep it in their pants, rescue you from a sinking ship, etc.). Just as important, however, is the ability to trust yourself and your own judgment when entering into a relationship; if you have too little confidence, you can sabotage your relationships, and if you have too much, you’ll make commitments that won’t last and will hurt like hell when they break down. Learn to trust yourself by gathering facts, observing carefully, and using common sense to judge your friendships and make smart decisions. Then, regardless of over or under-confidence, you’ll be able to love someone you trust and have trust in whom you love.

I am in a good relationship and have been now for a while (around 9 months). But none of my relationships seem to last more than a couple of years (I’m now in my 40s), and I worry that some of them I have sabotaged myself. I am at a point in this relationship where we have acknowledged that we love each other and have started making plans months into the future (nothing like moving in together, but definitely trips and such), and suddenly, I have this fear I’m going to lose him. But not just lose him—lose him to someone, and that someone is my friend. I had a friend when I was younger that flirted with my boyfriends, and even though nothing ever happened, it bothered me that she never understood these boundaries, didn’t have a sense of loyalty towards me, and used her looks and sexuality to get attention from those that should be considered off limits. Now I have a newer friend who is younger than me—she’s very pretty, smart, and single, and she has a tendency to try to connect with my boyfriend in ways that I am unable to by finding the gaps and honing in and I don’t like it. I am acting as though they have already run away together, or have a secret relationship. Is my own insecurity causing me to worry about this? My goal is to alleviate these fears of betrayal.
Having fun friends with fickle boundaries may damage your calm, but you do yourself more damage by letting them distract you from the real issues surrounding your boyfriend and your future together. Instead of worrying about whether your gal pals have good intentions, focus on doing the necessary homework to find out whether your boyfriend is a good match.
Assuming you’re not able to stop yourself from being insecure about your friends and boyfriends, use your insecurity to assess your boyfriend’s trustworthiness. Maybe you can also use it to get better at screening friends in the future, but for now, believe it or not, your best weapon against your paranoia is paranoia itself.
Instead of trying to feel better by talking about your fears and asking for reassurance, use them to review your boyfriend’s history with women and your girlfriend’s history as a femme fatale. Tough love is always a tricky option; it’s never clear when it’s appropriate, if it’s ever appropriate, or when you’re so fed up that you’ve actually crossed the line into “easy dislike.” This is especially true when you can’t seem to get through to a troubled loved one and aren’t sure whether you need to do more or “toughen up” and do less. Instead of letting fear or frustration control your involvement, ask yourself what prevents him or her from getting help, then try different strategies, and observe what happens. Sometimes more is more and sometimes more is less, but you can be sure, no matter how helpful you can be, that you’re doing your best with what in unquestionably a tough situation.

My brother is sinking into an economic mess and he won’t let me help him. He’s a good guy who’s worked for years at the kind of manufacturing jobs that are now being shipped overseas, and his last position was just eliminated. I’m good at managing problems like this and discussed his options—selling his place, cutting back on expenses, getting employment counseling—but he doesn’t follow through, or even seem to pay attention. Sometimes I think he’s got some brain issues or something, because he invited me over to dinner recently and when I showed up he was asleep on the couch after eating fast food. He’s depressed, but he can laugh and enjoy himself, so it’s more that he spaces out whenever he has to do something complicated. My goal is to get him to get moving before he goes deep into debt and can’t pay his bills.
Some people don’t respond to good advice because they’re stubborn or lazy, while others appear stubborn or lazy because their brains are failing to process information normally. There’s a big difference between having a damaged personality and a damaged brain.
The fact that your brother forgot he invited you to dinner suggests he’s having trouble with attention and maybe memory. The bad news is that he might have some serious cognitive issues, but the good news is that, with a little time and effort, you might be able to help him with his financial situation after all. WAIT! All of us suffer from a Cassandra complex at one time or another, where we see something so clearly—from how your brother will regret eating that gas station sushi to why your wife will wish she’d never paid for a year of intensive aerial Pilates up front—but can’t get anyone to heed our vision. Sometimes you can see a disaster looming because people are too angry and agitated, and sometimes it’s because they’re complacent and don’t give a damn. In either case, before you try to sound the alarm, give thought to the reasons for their feelings and the probable impact of your attempts to warn them so that your attempts to change opinions don’t accidentally cement them. Provide wise counsel if you can, but don’t expect anyone to listen until events, even painful sushi-related ones, put them in a more receptive state of mind.

As long as I avoid certain (mostly political, mostly right wing) topics, I get along with my in-laws pretty well. The problem is that such topics sometimes become unavoidable, usually when they get really wound up about a specific current event, and right now, they’re very vocal about being rabidly pro-Israel. I’m Jewish (they aren’t), so they assume I feel the same way as them and shower me with links and e-mail forwards that are nothing short of anti-Palestinian propaganda, but I don’t agree with them necessarily, and that kind of thing makes my skin crawl. They see the issue as black and white, and let’s just say I just see it as complicated, infuriating and heartbreaking, and their warmongering just angers and depresses me. I want to find a way to respond to them that gets them to see how damaging and foolish their angry rhetoric is, or to at least find some consensus in getting them to agree that killing isn’t answering anything. My goal is to get my in-laws to drop the subject in an enlightening, non-provocative way, even if I can’t change their minds.
One reason it’s hard to stop angry war rhetoric, particularly if it comes from nice people who aren’t particularly angry in non-political situations, is that they feel that aggressive action is necessary to prevent or overcome a dangerous threat. From annoying e-mail forwards to amassing an arsenal, fear rarely leads to thoughtful, positive action.
So if you suggest that your in-laws are advocating useless conflict with and killing of Palestinians, you’re questioning their morality at a time that they’re calling for moral sacrifice. Despite what the Ramones (R.I.P.) once declared, most people do not want to be sedated, especially if it’s for reasons involving “going loco.” Some people can’t think about psychiatric hospital admission as other than a form of kidnapping, and others as a failure that should never have happened if they took proper care of themselves. In reality, it’s good to think about psychiatric admission as something that can happen again regardless of how well you take care of yourself, and will rarely happen for reasons that you won’t ultimately agree with. The more you accept the possibility of hospital commitment and consider your own views about what makes hospitalization necessary, the more skilled you’ll be at managing the situation if it occurs again, even if it’s something you’re never going to wanna do.

I’ve got depression that is usually controlled well by medication, but I had one bad episode three years ago when I got really down, couldn’t leave the house for a month, and was on track to starve myself to death. My parents were right to pull me out and take me to the hospital, but it was a horrible experience; there were some scary, sick people there, and staying there was traumatizing. Now my shrink wants me to put together a crisis plan that will tell my parents how to decide when they should take me to the hospital, if it ever becomes necessary again—a sort of “advance directive”—and I’m trying to figure out how to make sure that I don’t have to go back unless it’s really, really necessary. The last thing I want is to visit an emergency room where they like to lock people up, so I end up trapped in the nightmare ward again. My goal is to figure out how to minimize the possibility that I will get admitted again.
As traumatic as it felt to be admitted to a psychiatric hospital, you are familiar with the bigger trauma that you would have experienced if you weren’t admitted. The scary people you say in the psych ward were probably fairies and pussycats compared to the hellscape that your own home had become.
You know how painful your depression was, how it interrupted everything important in your life, including work, relationships and your ability to care for yourself, and how it endangered your health and your life. Whether you’re humiliated by excess praise or criticism, too much attention for one’s deeds, be it positive or negative, can make you feel fake and, worst of all, stuck in a world where good deeds get punished. Instead of trying to make sense of this classic disconnect, accept the stupid way life has of making innocent, hardworking people miserable. Then continue, as before, to work hard for what you believe in while ignoring pain that often comes to those whose achievements are special and need for recognition isn’t.

A year ago, I was named by a hospital journal as an outstanding up-and-coming oncologist, and now my name is synonymous with accusations of malpractice. I didn’t do anything wrong, but I made the mistake of pissing off a couple crazy nurses, having bad luck in the operating room, and being unfriendly with reporters. I won’t get convicted, but my reputation is shot, I’ve got no patients and huge legal bills, and most of my so-called friends don’t answer my calls. I can’t get out of bed and, when I’m up, I can’t focus on getting anything done. My goal is to figure out how to get my life back.
Being a star physician isn’t much different from being a celebrity; praise and attention make you feel like you’ve made the right career choices and are good at what you do. Unfortunately, power attracts its own special kind of bad luck, and now you’re the medical world’s Justin Bieber.
When you’re up and coming, people seem ready to ignore your faults and exaggerate your virtues. After a while, however, you become one of the powers that be, which people like to tear down.
