Anxiety and depression can act like funhouse mirrors, distorting your thoughts and making it hard to perceive the benefits and obligations of social relationships. Some people can’t say no to social responsibility, no matter how unreasonable, and some people can’t say yes to it, even when it’s minimal. Regardless of the distortions that make you over- or under-embrace your social pleasures and responsibilities, use objective methods to figure out how much is good for you and how much is necessary. Then you can build a social life and discharge responsibilities without burning out or drying up, and substitute the funhouse for actual fun.

My elder brother was diagnosed with MS when he was in his 30s, and now he’s in a nursing home and a wheelchair. He’s given me his health care proxy and told me he was relying on me, calling me five times a day and giving me a hard time if I don’t answer, so now my anxiety has gone through the roof. He’s always been a spoiled brat and now it’s worse. His health care is actually pretty good, and there are people and activities in the nursing home to keep him occupied, but he expects me to drive 100 miles to visit whenever he asks. I’ve always been the one everyone turns to for help, but now I go around with a knot in my stomach and desperately need to take more medication, even though I know it’s addictive and likely to make me fall. My goal is to find some way to relieve my anxiety that doesn’t kill me.
If you’re an anxious person, then there’s no substance in the world—pill, liquid, magic bean—that can make you suddenly be a not-anxious person. Especially if you continue to do things that feed your anxiety, like say yes to anyone who needs your help because you’re too anxious to say no.

Maybe it’s the eternal bond that makes it hard to walk away from a conflict and impossible not to start a fight in the first place, but when you throw down with family over a fundamental disagreement, it’s almost impossible not to be either beaten down or drawn into a fight. So, regardless of how uneasy it makes you or how much you want to get into it, learn how to stop talking, politely, when you feel further conversation will do you no good. You may be condemned for doing so, but only by others in the clan; the only thing you’ll be condemning yourself for is getting into it in the first place.

My parents know my brother and I both have bipolar illness, and they’ve seen how stable I am on my medication. My brother, on the other hand, pretends he doesn’t really have it and refuses to take medication. Then, when he gets hospitalized, they don’t disagree with him when he blames it all on stress and bad marijuana. After his last admission, I wrote him a letter begging him to do what I do and be careful about taking his meds and getting enough sleep. His response was to tell me he resented being called bipolar, and tell our parents that he resented me, period, and now they want us all to meet so I can explain to him I didn’t really mean it. I’ve had it with all of them. My goal is to not have to deal with him, his illness, or my parents’ co-dependence, ever again.
In many ways, your brother is like the princess that needs saving at the end of a video game; both you and your parents have set out to win and free him, but you’re all stuck on different levels for different reasons, all equally unlikely to get to him anytime soon. As games go, it’s not exactly a bestseller.
Unlike most Nintendo princessi, however, your brother is not passive and helpless, but he’s fighting to stay unsaved, mostly because he doesn’t actually acknowledge he needs saving. Despite all the levels of difficulty that come with this specific quest, that doesn’t mean you should get fed up and walk away.
Certainly, you have a right to be angry, but your feelings are relatively unimportant compared to your desire to help your brother if possible, while staying out of conflict. Everybody has a limit as to how much they can bear to differ and share with their partner; it’s just as difficult to be with someone who agrees with you too much as with someone with whom you can never see eye-to-eye, no matter what James Carville and Mary Matalin say. Distinguishing between acceptable and irreconcilable differences is tricky; some differences need not threaten a partnership, but it’s not good for either of you to stay together if one of you believes the other is bad and beyond redemption. So weigh everything you know about a person’s reliability and loyalty before deciding what to make of your ethical, religious, or cable news differences and your partnership.

I think my husband is much happier about our moving to a new town since he joined a local church, and I thought I’d feel welcome there too, but I was not pleased to hear that they recently expelled a woman from the congregation because she’s lesbian. My husband believes it was the wrong thing to do, but he loves the way the congregation makes him and others feel valued and at home, so he’s not about to let go of his new friends. I’ve objected publicly, and no one argues with me, but they treat me with kindly disagreement, making it clear I’m welcome even if I can’t understand where they come from. I still have trouble understanding how my husband can be OK with this under any circumstances when in my eyes it’s bigotry, plain and simple. My goal is to honor my principles while not letting the church come between me and my husband.
Although there’s an apparent difference of principle standing between you and your husband, you have no reason to doubt his honesty, fidelity, or any other character issue that would interfere with his being a good partner. The difficulty is reconciling that with your new impression that he might not be a good person.
You’ve been together, I assume, for a number of years and have weathered storms, so you have good reason to respect him as a person and husband. You don’t expect his continued membership in a church that rejects gays and lesbians will make him less honest and supportive. If you could ask Mary Cheney or A.J. Soprano, they’d tell you that inheritance, be it material or psychological, is always tricky. That’s because it’s easy to hate yourself when you can’t get rid of an inherited personality trait, like constant anger or depression, which makes it hard to ever feel happy or express love and affection. What life teaches you, however, is that many people find ways to take care of one another and contribute to the world, even when they’re not fully functional or in full control of their dark thoughts and sharp tongues. They deserve respect for whatever good they do when their feelings give them no relief or reward, and their genes don’t give them much of a choice.

I grew up in a horrid family. My father worked long hours at a couple of jobs leaving my (very young) mother alone with my brother and me. My mother had no clue about parenting and raising children. My home was a miserable cesspool of put-downs, depression, negativity, and yelling. I get that my mother probably grew up herself with a “Mommy Dearest”/”Carrie” mother, but as we all do, I vowed to myself never to torture my children as my mother had done to me. But guess what, I now find myself yelling and haranguing my daughter (but not my son…hmmm?). I think the yelling and disrespectful attitude toward my daughter has been programmed into my DNA. It is my default reaction when I get angry (or when my daughter does anything–wrong– pretty much). I’ve done the anger management, self-help books, etc. I can’t stop. Please give me some advice on how I can stop the “bad psycho mother” cycle. I already see the signs of damage in my daughter.
It’s painful to grow up with an angry, critical parent, but it’s even worse to grow up with a parent who can’t provide for his/her family or care enough to try. Despite the pressures on your immigrant parents, they were able to survive and provide you with a home. Even if it was horrid, it beat the alternative.
So, even if you see yourself as a bad psycho mother, you’re also a caring, providing mother, and you need to remember that. That’s why you need to respect what you do right before picking at what you do wrong, because if you can’t see the big picture, you can’t get at the big issues with your parenting. WAIT! As the main motive for most of life’s poor decisions—shotgun weddings, gift cards as birthday presents, matching tattoos—guilt can cause us to avoid responsibility that is ours and impose responsibility that’s undeserved. So don’t let your conscience be your guide until you’ve carefully considered what you actually control, rather than feeling bad because there’s pain and you’re involved. Then, if your conscience won’t listen to reasonable judgment, learn to ignore it and pay more attention to what you believe is right instead of what makes you feel wrong.

I don’t know how to help my son deal with a crazy high school relationship. He’s been dating a very troubled but pretty girl who now says she’ll kill herself if he ever breaks up with her. He’s a sweet kid and always likes to help people, but he also feels drained by all the attention she requires and he tells me he really would like to break up with her. I think it’s an unhealthy relationship and I’m delighted he’s ready to move on, but, like my son, I’m worried about what will happen if she tries to kill herself. I can’t speak with her parents because my son won’t let me—he says that would break his promise to her and make it even more likely she would hurt herself. My goal is to figure out a way to protect my son and this girl.
Unfortunately, whenever a soon-to-be rejected, needy lover threatens suicide, there’s no way you can protect anyone from pain and potential guilt. It’s effectively a hostage situation, which means, by design, it can’t end well for everyone, will always end badly for someone, and may well end with irreversible disaster.
No matter what happens, your son’s girlfriend is going to get hurt and maybe hurt herself, whereupon your son is going to feel guilty, and so will you, if your son accuses you of violating his confidence. So forget about who’s going to suffer for what and just focus on doing the right thing and getting everyone out as safely as possible.
Your first priority, of course, is doing what you can to reduce his girlfriend’s risk of self-harm. If her parents, shrink, and/or school officials know she’s at risk and are monitoring her closely, then there’s nothing you can add and nothing further to do, but the only way to find out is to tell them. It’s true, your son might not approve, but you have a duty to make sure those who care for her know what they need to.
Don’t expect to stop feelings of guilt simply by doing the right thing. All it takes to feel guilty is to be the type of person who feels responsible for the feelings of others—a description that fits most of us shrinks—or to be told by someone you care about that you’ve made them suffer, disappointed them, or let them down. For most of us, guilt isn’t rational and there’s no escaping it. We may try to feel better by bending over backwards, but this usually just causes a sore back, a bigger sense of responsibility, and even more guilt.
What you can do, however, for your son and yourself, is not accept that this guilt is deserved. Begin by asking yourself, and your son, how much responsibility a loving person should take for the feelings of someone who is needy and sensitive to rejection. You’d like to think that your love can protect them, but unfortunately, it doesn’t work that way. The more you nurture them, and the closer you get, the greater the chance you’ll trigger their sensitivity, and then it’s all down hill. It’s no one’s fault, so it’s best you keep your distance until they develop tools for managing their sensitivity, if that’s what they’re able to do.
Do what’s necessary to protect his girlfriend’s life, and give your son the tools to judge his responsibilities apart from guilty feelings. Initially, of course, they will control how he judges his actions and yours. You can then show him, however, that, regardless of guilty feelings, you have better methods for making decisions that make the best of situations that can’t be good for anyone, and that he can learn your methods when he’s ready.
Learn to negotiate, not just with his emotional captor, but with your own emotions, and with luck you can help everyone emerge safely.

“I hate to think how my son is likely to blame himself, and possibly me, when his girlfriend blames her breakdown on his dumping her, but this is a life dilemma he needs to learn how to deal with. I will show him how to use ethical reasoning to define his actual responsibility, apart from guilty feelings, and do the right thing.”
I’m having trouble getting over my mother’s death in a car accident because it was so sudden and we never had a chance to make up after a nasty argument the night before. She had a fiery temper, and we had a stupid argument that really meant nothing, but I hung up on her while she was yelling at me and I can’t stand the idea that that was our last interaction and that the stress of our fight may have caused her to drive mad and get into the accident that ended her life. My goal is to find a way to live with my guilt.
As noted above, guilt seldom has anything to do with actually doing wrong; if you feel guilty about your mother’s death, it’s because people usually feel responsible for protecting those they love, whether or not they actually have the power to do so. It’s an instinct that probably helps us look out for one another and is thus mostly helpful, except in situations like yours, when it can tear you apart.
Instead of trying to ease your guilt by kicking yourself, ask yourself how you would weigh a friend’s responsibility under similar circumstances. Give your friend a small share of blame if he was particularly cruel to his mother before her death, but give his mother responsibility for managing her own sensitivity, protecting herself from hurt, and controlling her anger.
Then think of how you would like to be remembered after your death; not by the circumstances of your last few years, days, or minutes, but by the sum total of what you built, who you were, and the good things you left behind. So spend some time assessing the value of what your mother did for you, and you for her, during those times when you weren’t having screaming fights.
Write out a statement that does justice to your whole relationship. Don’t try to diminish guilt through apology or confession, just ignore it by honoring values that are more important and using them to build a view of your relationship that is truer to what matters. The unexpectedness of her death shouldn’t teach you to avoid ever being mean, but to remember that life is short and most aggravations don’t really matter.
Honor your temper and where it came from. Remember that arguments never really drove you and your mother apart, as painful or stupid as they were, and they never really interfered with your relationship, which could only be cut short by accident and death. Now, your job is to prevent guilt from interfering with the relationship that you will continue to have with her for the rest of your life, and to protect yourself, and that bond, from being devalued.

“I can’t help feeling guilty over not having made up with my mother before her death, but that’s not how I really value our relationship. I will cherish her memory for the things that mattered, and carry on what was best about her values, while ignoring guilty feelings that I can’t stop having, but that are unimportant.”

Many of us suffer stress and torment by accepting nutty ideas, either because we’re literally hearing voices, or just because we’re reading women’s magazines or buying into our own baseless guilt. It’s easy to feel you’re doing what you have to do when you’re really just holding yourself responsible for problems you don’t control and making efforts that can do no good. Unless mental illness makes it impossible, most of us should examine our beliefs before accepting them. Then we’re better able to stand up to critical thoughts, undeserved self-punishment, and airbrushed models in the name of deeper values.

My sister started hearing voices when she was about 20 and then got diagnosed with schizophrenia, but she controls it well with medication and is able to hold a challenging secretarial job, so I know she’s relatively lucky. Sure, she had a brief hospital stay a couple years ago when her paranoia got out of hand, but since then she’s been fine. The other day, however, when we had dinner together, she was more outgoing about her fears of my being able to read her mind or put thoughts in her head. I was glad she could confide in me and I wondered if that was a sign she was getting better, but then I had second-thoughts about whether, if she was talking about it more, that it was maybe getting worse, and she was going to share her fears inappropriately with people at work. I’d like to know whether her talking about her symptoms is a good sign or bad sign and what I should advise her to do.
Back in the day, shrinks always thought sharing was a good thing, even if patients shared how much they hated us, thought we were aliens from Jupiter, and/or wanted to kill us. Thanks for sharing, even more thanks for not murdering.
In the long run, we thought sharing was always a good step towards recovery. Actual experience, however, has taught us otherwise, so your question is sensible, and you should, indeed, be prepared to discourage sharing when you think it’s a bad idea.
The key question you should ask your sister, and encourage her to ask herself, is whether she’s as sure as she usually is that the things she fears aren’t really happening. Ironically, the two simplest ways to make a problem worse are also entirely contradictory; you can give the problem too much attention, or pretend it doesn’t exist. That means the ideal, middle ground is finding the courage to admit you have a problem, particularly if it’s stigmatized, then summoning the additional courage to put it out of your mind after doing your best to manage it. It’s not a simple process, but it is the best way to ensure that you both face your problem and not let it take over your life.

I was just told recently that I have ADD. Lately, I’ve been wondering if my brain has other irregularities (I’m pretty sure it does, I can barely do basic f*cking math). I try to bring it up to my relatives, if I maybe have other learning disabilities, but they say it’s genes or it’s just my ADD. I end up feeling like a hypochondriac or just plain crazy. I guess my point is, how do I go about trying to convince my relatives that maybe there is more to know and get their support? Although even asking that is making me feel like a hypochondriacal nut-job.
Contemplating your brain can be just as useless as contemplating your navel; although the latter is usually a euphemism for useless, time-wasting self-involvement, aimlessly exploring any body part is a huge waste of time.
Obsessing over a disability can make you feel helpless and inadequate, so before trying to learn more about your brain, ask yourself why you want to know. If you need to know more about your brain problems in order to manage them better, more power to you. Otherwise, accept the ways your brain is broken and find a work-around. WAIT! We’ve said many times that people who loudly, repeatedly declare they’re good or bad at something are often wrong; for example, a self-proclaimed expert cook should not be trusted to boil ramen, and someone who makes it clear they’re a terrible dancer just needs a little prodding before they unleash the boogie. That’s why you should always look deeper when someone says they’re awful or great at parenting, because some good parents are always sure their unloving feelings are messing up their kids when they’re not, and some obviously-not-so-good parents believe that, since they love their kids and love is all important, they’ve got what it takes when they don’t. Instead of rating yourself on the love vs. let-me-out-of-here scale, describe the behaviors necessary for the parenting job and grade yourself using whatever objective feedback and observations you can gather. Then you’ll be able to assess yourself accurately, both in your own mind and in public statements.

I’m pregnant! No celebration though, because I want to terminate the pregnancy. When my husband and I got married, I said I’d have babies—he wanted them, and I always thought eventually I would. I kept putting it off because the biological clock never started ticking, and the thought of motherhood still freaks me out. I never lied to him on purpose, I always thought the mother instinct would kick in at some point because everyone said it’s what women do, become moms. We started “trying” because I couldn’t put it off any longer, although I did everything I could not to get pregnant (sex on non-fertile days, etc.). I know it was stupid, but I love my husband so much, I couldn’t not try. Now that I’m pregnant though, I want to scream. I pray everyday that I miscarry (it’s still early). I know I’m an awful person. He wants this so much and I love him so much. I just think I’ll make a horrible mother and am too selfish and career focused to make a good parent. I want a life, not a baby. What should I do?
Just because the idea of motherhood freaks you out doesn’t mean you’d be a terrible mother. If every med student who got freaked out by his or her first scalpel-cut into a cadaver decided s/he was no good at doctoring, the medical profession would be a very small group, made up mostly of cold-blooded serial killers.
Freaking out during your first pregnancy is normal—fear helps you prepare for parenthood—and there are many good reasons for believing your worst fears won’t come true. As emotions go, guilt is often the most devious; it’s very good at tricking us into believing we’ve done something wrong, when we know we haven’t. It’s so slick that you can feel guilty for not feeling guilty in the first place. That’s why guilt is a terrible barometer for whether or not you’ve made a good or bad decision; sometimes the right choice makes you feel wrong and the wrong one righteous, at least in the short run. If you think of values and consequences, however, as well as needs, you’ll usually come up with a choice you won’t feel guilty about being proud of.

I’m a medical doctor—I graduated three years ago and did extremely well in my school. But then I broke up with my boyfriend, and for a while, I hated myself very much for it. Coupled to it was the death of one of my closest relatives. I turned lethargic, did not want to function, never talked to anyone and was alone. Finally I managed to get into a job not at all related with medicine, but for the past few months, I have been experiencing severe guilt about my decision. At the same time I don’t feel active or ready enough to put in the hard work necessary to go back to medicine. At least this feeling is welcome as I am finally feeling something. I have stopped trusting any guy and don’t want a relationship, but I’m consumed by this feeling of guilt. Unfortunately I also have a bit of narcissism, I feel that I m more intelligent than most of the people I meet, and so did not visit any counselor either. I just want to know if you have any suggestions on how to conquer my guilt and lethargy.
Even with support from a very nice therapist, all the insight in the world, and/or a job in the medical profession, guilt and lethargy often hang around as long as they want. You could find cures for cancer, bad boyfriends, and even depression, but still feel guilty that it took you so long, guilt is that insidious.
The way to conquer those symptoms is to prevent them from affecting your beliefs about yourself and the world. Up until now, it seems the symptoms have been brainwashing you into feeling like a hateful, worthless failure instead of a smart medical school graduate with an unlucky personal life. WAIT! While most people with mood disorders don’t technically “hear voices,” they sometimes experience something much scarier; their own internal monologue lying to them. Whether your mental illness is convincing you you’re worthless or the king of the world, don’t follow your corrupted instincts when it comes to managing medications and drawing up safety plans. Instead, study the facts, learn from your own experience, and plan the odds, rather than letting your confidence play you and make you crazier than you really are.

I am a doctoral candidate preparing to defend and graduate in May. I am so terrified of what comes next (giant black hole; there be dragons) that I am undermining my progress and succumbing to despair. This is tied to my utter lack of ambition. I don’t know how to dream Real Things. All this worry and fear and lethargy leaves me feeling exhausted. And I am so worried and so frustrated with (what I perceive to be) my laziness, my incompetency that I become angry at the smallest things. How can I learn to get work done and to build a life? How can I learn to do it just because it must be done? How can I distinguish between real and false selfishness (it feels selfish to build a life just for me)?
The business of many doctoral candidates is to find a meaning in many random, painstakingly-researched, frustrating phenomena that ties them together and gives you a new idea you can dwell on for several hundred pages. Congratulations, you’re very good at it; too good at it, as you’ve turned your talents on your own troubles.
It’s certainly possible that your fear of any and all future jobs, combined with terminal laziness, incompetence, and an inability to distinguish between real and false selfishness have paralyzed you and filled you with self-loathing.
